Care Coordination

(703) 670-1336; (540) 659-1800, Ext. 1336

Being hospitalized can be a difficult, confusing time. Sorting out physical and emotional problems due to an illness and making sense of information and choices can be overwhelming.

Potomac Hospital's Care Coordination Department is available to help hospitalized patients and their families. We can meet with you to discuss your concerns and guide you in finding solutions. Our Care Coordinators have extensive experience and skills in assisting people within the medical system.

For example, you may need continuing care and special assistance at discharge; someone from our department can work with you to plan ahead for this possibility. We want to make sure you leave the hospital with the knowledge and resources you need to feel confident about managing your healthcare needs.

Our services are available at no charge to patients in the hospital. If we can be of service to you and your family, please ask your doctor, nurse, or any hospital staff to make a referral.
