Welcome to the Potomac Hospital team. Occupational Health Services (OHS) is here to assist you before your first day on the job begins and thereafter.
As an important step in joining our team, please schedule an appointment to meet with the OHS Nurse. To make your appointment, please call (703) 670-1338.
To assist us before your visit, please complete pages 1-3 of the Pre-placement Health Assessment Form
At Occupational Health Services, you will:
- Complete a pre-placement health assessment where your medical history questionnaire will be reviewed. Please review your immunizations history (bring records, if available) and past medical history.
- Be fit-tested to wear personal protective equipment if necessary.
- Receive a TB test (a chest x-ray will be performed if TB test has been positive in the past).
Human Resources will be notified when all criteria of pre-placement health assessment have been met and they will notify your Department Director. We look forward to seeing you soon.
